I gotta get out of this job – help me figure out how

I have CPTSD (and am on SSDI (but not getting any money bc I'm working)), and its effect on my memory is causing me to fuck up at work. My memory is shitty and sporadic. I have no control over what I'll remember and what I won't. Unfortunately, I'm not remembering to do all the little necessary details of my job that need doing. I have ADHD too (am trying to find the right med with my Dr) and my focus is everywhere but never quite where it needs to be. I got a written warning at work for not being “perfect” (it's required for what we do) and have 30 days to step it up. I have CPTSD (and am on SSDI (but not getting any money bc I'm working)), and its effect on my memory is causing me to fuck up at work. My memory is shitty and sporadic. I have no control over what I'll remember and what I won't. Unfortunately, I'm not remembering to do all the little necessary details of my job that need doing. I have ADHD too (am trying to find the right med with my Dr) and my focus is everywhere but never quite where it needs to be. But reading what the warning said, and listening to my boss' expectations for me, I have realized I am in way, way over my head. I do not have the necessary attention to detail, memory, or focus for this job w this group despite my 10 years' experience. However I LOVE the company I work for! The benefits are amazing and everyone is kind and all work well together. That said, one can only change positions w/in the company every 2 years. I've only been in my position 9 months, so I can't get a different job w/in the company.

Here are my options:

1 – try to do better even though I know that even if I am perfect in what I am doing right now, I will 100% fuck up the things I'm being groomed to do in the future bc there are so many more steps and so many more details.

2 – look for a new job, hand in my notice when I get one explaining why, leave.

2a – tell my boss now that I can't meet his expectations and I'll be looking for a new job, allowing him time to find someone to fill my position before I'm gone.

3 – potential nuclear option: when I applied for my job, I checked that I am disabled (am currently on SSDI (but not receiving money)). I can involve HR and let them know my disability is preventing me from being able to provide exactly what it is my boss is requiring now and in the future and how should we proceed? There's no accomodation that can be made to help me bc I already WFH and have tons of memory aids, but there is just SO MUCH to remember. The only way HR can help is if they can put me in another position in the company. But given then 2 year rule… HR people, can you speak to this: in this type of situation, can someone be moved to another position in the company, despite the rule?
