employer wants us to declare appointments outside of shift times

this isn’t about me, it’s my co-worker who has a heart condition. today, at the beginning of the shift they told him he has to stay for an extra two hours at the end to cover the other shift, he said he couldn’t because he has a doctors appointment at 3 (roughly 45 minutes AFTER our shift ends) this isn’t a regular occurrence so my co-worker had no reason to tell them about his appointment but they’re adamant we should tell them “in case”. I think this is ridiculous. they’re chastising him for not telling them in advance yet they’ve known about this required overtime for a week but only told him this morning and think they’re being courteous by not waiting until the end of the shift
