How to preserve vacation days when leaving?

Advice needed: I work for an AH owner of a very small company. I'm in salary negotiations with a company I've been trying to get hired on for six months. I'm not sure how to proceed with leaving because of a few issues. I have a vacation planned for the end of the month. If I give my notice before vacation, I'm afraid AH won't allow me to go as planned. If I don't go, I forfeit my vacation days and pay when I leave. Assuming I want to start at the new company shortly after I return from vacation, how do I handle this? It isn't necessary to give a full two weeks notice; here one week is acceptable. I'm afraid I can't see the forest for the trees here.
