Advice: likely to lose my job during my probationary period due to being ill

Live and work in the UK. So I got this job working remotely at a bank about six months ago, mainly because I worked in the private mental health sector for the last 12 years and lockdown completely destroyed me. When I took this new job, I was told it was remote and you may occasionally need to come to the office, which is fine. It's very difficult because I've developed agoraphobia and I've also lost my license because of a possible medical issue, the two sort of go hand in hand. Once they've looked in to the medical issue, apparently I can then drive again. I've also recently had to move an hour away from the job and again told this was fine, and I may need to come in once in a while, again, no issue with that at all. I've also recently had to move an hour away from the job and again told this was fine, and I may need to come in once in a while, again, no issue with that at all.

Anyway, fast forward to today, I'm on annual leave and one of the managers message me, saying I need to come in this week for my probationary review. I ignore saying that I'm on annual leave, and explain that I'm currently in (new town) and could this be done remotely? Also, that due to my medical issue where I'm randomly passing out, travelling by public transport is a bit daunting at the moment. Just to add, this was cleared with the Operations Manager as being no issue (me moving an hour away).

I get a reply saying that yes, it can be done remotely, but that we will have to review your position at the company due to your circumstances. For some bizarre reason, we can only message managers on Slack, so any reply from me looks ridiculous, like I'm having a text argument with a manager (also, the chat is filled with all managers and higher ups, so like ten people are in my chat).

From my perspective, they're framing me saying “can the review be done remotely due to blah blah blah” as, “you're refusing to come to work”.

I highly anticipate that when my review is done this week, that I'm going to be let go. Even though my stats are apparently in the higher tier.

What do I do in this upcoming review? I feel like I'm going to be forced out and want to be able to at least reply with some knowledge rather than being dicked over by comments like “it's just not working is it” or “you had a few sick days”. Yes, I had a few sick days because I kept passing out without reason.
