What do I do? Never dealt with WC

So, I’ll preface by saying that I’m at a loss. I’ve never been in this position before and I don’t know what step to take. I’ve been on WC since mid November. At first it seemed like the company was good people. They wanted to help me get better and move back to my position. I herniated a disc in my lower back and it impinged on my sciatic nerve. After several months, they got to be tired of paying me so much for not being able to do my duties and having very limited restrictions. (It’s a very toxic environment) I started making significant progress in PT and low and behold they decided to have me do something outside of my restrictions. Guess what… my back got re-injured. Today I got a call from insurance. Insurance offered me a settlement and the ability to move on from that company but they will no longer be covering any medical if I choose to accept. I also found out from insurance that the company had been “spying” on me when they said I had been lifting my dog (which I hadn’t) the company had been lying to insurance about me not showing up lately when I was told not to come in until the new return to work order was signed off on by the doctor (still hasn’t). They are manipulative and layers and all together just shit humans.
So here’s my question:
Do I get a WC attorney? Do I stay (like I had planned) until I get off WC? Do I get a consult from a lawyer? Or do I just accept the offer from insurance?

Also this company has been trying to make me leave since December rather than firing me. I havent let them bully me or pressure me into leaving on my own accord but I’ve had it with them. I just want to be done with them but I don’t want to loose WC benefits and have to pay for medical on my own. Any direction from someone who has been in a similar place would be very helpful.
