Interviewer requests I take a “survey” before interview

She didn't explain what the survey was, except it “gives us a sketch of your motivations.” I was willing to take a look, but the form required a signoff first, where the fine print was like full hold harmless for them and “any of our business partners involved in the hiring process.” The instructions also said the questions might seem “nonsensical” but that was okay. I said no, then wrote back to the person and politely explained I don't quite know what it is, but it seems like a psychological assessment that could be used against me, so I decline to do that. I did not withdraw from the job, but I assume I am toast.
