Restricted work hours with doctors note

So I'm 21f with aspergers, depression and anxiety working 40 hours a week currently. Does anyone else feel like they can't work 40 hours a week? I feel kind of pathetic admitting it, but it genuinely is just terrible for my health, emotionally, mentally and physically and makes my depression so much worse. I'm seriously considering getting a doctors note from my psychologist to limit my work hours to 6 hours a day, 5 days a week. I don't know much about the process so I have some questions. First, if one were to leave their job and plan to work somewhere else (and they don't have part time), would one have to bring up during the interview that they can only work part time? Wouldn't it also be possible that, once hired and you share the doctors note with them, they can just fire you?
