My company reported my injury as its my fault when I’ve never once said what they have written in the incident description. Do I talk to the company to correct it? Who do I talk to if they don’t budge?

They altered the incident description to make it look like Im responsible for the injury. My description of “the door doesn't stay open and comes back to close is how I got injured” is changed to “didn't open the door full and backed into it”. PoS Cooperates. This is in Victoria for reference.
