Hi, trying to take a mental health leave and my employer is asking me to fill out EFT Request Form 2438A(6-2020), Reimbursement Agreement and EFT request form…seems strange that they want to withdraw or transfer money to my bank account. Is this normal? Resides in Houston, Texas
