I am in Saskatchewan Canada just so people know before giving advice.
So i work at a small liquor store. I injured my wrist at work and the sequence of events went like this:
– injury
– called Manager (Alex) and said I needed to leave to see a doctor
– went to a walk in clinic and got a “Medical Restrictions/Capabilities” form from the doctor that states my injury, return date, and accommodations when I go back to work. The doctor signed it. The doctor also filled out the WCB stuff online so I didn't have a paper copy.
-I passed a photo along to my manager and went home.
- I fill out my W1 online and call the clinic to make sure my doctor put his paper work in my online file (he did).
- the next day my manager says she needs another form “from your doctor stating your time off. It has to come from their office with your sticker” (I still don't know what sticker she is talking about.)
- I called the clinic and they said the “Medical Restrictions” form was a note because the doctor signed it and had the information on it.
- I told my manager what the clinic said.
- “you did not provide me with a doctor's note. That just had the name of a doctor and your name on a work safe form”.
(I'm getting pissed off at this point, my hand is in pain and I just want to sit at home with ice) - I call my clinic and they get me another form. They give me a “Certificate of Illness” that includes: the clinic and doctor saying I was there, and the doctor saying to look at the “Medical Restrictions” form for return date.
- I send a photo of this second form to Alex. (Who doesn't respond) and leave a copy of both in her office on my way home.
Everyone I have talked to said my manager is draging me around. I am technically scheduled tomorrow but the doctor said I need until Monday off. I am panic because this job pays well despite everything.
Have I missed something here?
