Seeking guidance in a formal response

Today I received a survey monkey link from my former employer several months after a very messy departure. In my reply, I asked her why are they curious about obtaining my feedback and the response comes from an exec stating that there was a lapse in processing. With that being said, since my severance the following has occurred with my interactions with HR: Never received confirmation about corrective action taken towards my manager even though they sympathized and promised a response. It took many weeks and continued hand holding for my approved retroactive reimbursement check to be processed My termination letter included incorrect information addressing my PTO pay out. When I reached out to inform the department about the issue, the employee told me that a reissued letter was emailed to me despite zero record on my end. With that being said, since my severance the following has occurred with my interactions with HR:

  • Never received confirmation about corrective action taken towards my manager even though they sympathized and promised a response.
  • It took many weeks and continued hand holding for my approved retroactive reimbursement check to be processed
  • My termination letter included incorrect information addressing my PTO pay out. When I reached out to inform the department about the issue, the employee told me that a reissued letter was emailed to me despite zero record on my end.
  • A portion of my final pay was deposited into my closed retirement account.
  • My ESOP termination letter was mailed to my previous address despite my address change prior to my departure and this error was only discovered after I reached out to the department right before the distribution deadline.

I know that no response is a response. But I would be grateful for any recommendation make the best use out of this opportunity. Thank you in advance!
