I need help composing a complaint response to my employer, would someone with an Apple device be able to help?

I am not good with writing, as the title suggests, and am trying to sound professional. Communication and formality isn't a strong suit.

I am being discriminated against for my disability and this submission is my last one. I've had 4 others.
