Is there a secret “do not hire” list that HR people use or am I just incredibly unlucky?

I work at a terrible company that holds nepotism and narcissism as their core values. I’ve literally been told that the reason we are successful is because “we only hire people we know.” I’ve spent the last year and a half applying and networking and reapplying, trying to find my way to something else. I’ve redone my resume 5 times, reformatting and rephrasing every bullet point. I’ve applied to hundreds of jobs, often having had a conversation with someone inside the business beforehand. In all that time I have only had 3 interviews and never gotten past the first round. I feel like there has got to be a master list of people not to hire somewhere and somehow I got on it. I am going crazy trying to figure it out. Has anyone heard of something like this? For context, I am applying within my field for jobs which…

For context, I am applying within my field for jobs which I definitely qualify for which makes this incredibly confusing.
