Need help filling out the bits for a doctors note after I had a car crash. My bosses dont believe me because I have no marks, but it’s only by luck and fate that I did. My car was nearly a ball.

I dont know where else to go from here. I want to include;
-There are no physical restrictions and to take breaks when necessary.
– Was released 3-21-2022
– Stayed for guarded prognosis

I have all the info and the templates and everything, I just cant pull the words together for it to be believable.
