Separation Package
Your accrued vacation will be paid on your final paycheck. You are also eligible for $1,976.80 in severance pay pending your acceptance of the terms and conditions in our separation agreement.
The separation agreement is a voluntary offer by XXXXXX to provide you with compensation beyond your final pay. The separation agreement will be sent to via DocuSign on 5/31/2022. Severance is not included on your final paycheck but will be processed within 21 days of receiving your signed separation agreement.
If you are currently enrolled in our group medical insurance, elect COBRA in a timely manner, and execute the separation agreement, we will pay for your first month’s COBRA premium. If you elect dental COBRA, you will be responsible for the premium in its entirety. You must sign the separation agreement to qualify for your lump sum severance payment and medical insurance COBRA subsidy.
