PTO requesting

I want to know everyone's opinions on the PTO requesting time limits. Recently, my manager said that now we have to request PTO exactly 30 days in advance or unless it's a special circumstance, it'll be denied. 30 days, though? idk, that seems kinda crazy to me. ALSO, they want us to give the reason why we are asking for PTO off. Like what, I can't take a day off just for the sake of it, but I have to lie and say, “Oh well, I have a doctor's appt,” or something like that. It's PTO, it's already mine… Anyways, how far ahead do you think is acceptable to give notice for PTO?
