Told my work I was having mental health issues. Can they sue me?

I have been depressed and admittedly not performing like I should be. I work from home and it has been hard to stay motivated. My boss wanted to have a meeting about my under performance and I told them I was having some mental health issues and was dealing with them and trying to get back and reorganized. Today they sent me a lengthy form to get signed and filled out by my doctor. “Medical Certification of fitness of duty”. I’ve only been at this job since Feb. I’m worried this form will somehow result in them trying to sue me for back payment of salary. My boss (the owner) was very angry and didn’t take the news of my mental health issues well. He made mention in our meeting that I had misrepresented myself in our interview, which I did not, and that I’ve “taken” $85,000 from them. I…

I am absolutely qualified for this job, I just need to get my life back in order. Do I go through with this form? Is this something they will use against me? Do I continue to try to buy time with this company? Or do I resign? Will they still come after me for payment of my salary if I resign?
