When the investigation took place they never accounted in that Carlos was late that day for work because he had locked his keys in his truck he said.
When he arrived the supervisor rode his ass about being late and was yelling at him. Nightshift had set up the vessels differently than usual compared to what Carlos on day shift was use too, he was angry at the foreman as he started his shift, he was just really having a bad day
Because he went into work hot headed for being yelled at over something as ridiculous as being late, he wasnt concious of his new surrounding. He was putting a head on a vessel which was being done on the manual rolls and the head was too heavy for the vessel so they had put a chain around the rolls to hold it down but it lifted the whole thing, it lifted the rolls as well as the vessel.
He took the weight off the crane and it flipped up and crushed him, he was pinned between two vessels….
He was alive for awhile, screaming that he didnt want to die, begging others to help him, to not let him die.
Exchangers brought in a grief counselor the very next day, which, most of the workers were not told about and she was only there for the day to talk with the 50 different workers if they needed.
Not that anyone felt comfortable talking with her on company time because foremen usually yelled at you for even going to the washroom.

Ed was green, he was really young, inexperienced, just learning.
He wasnt comfortable with a task that was given to him, so he mentioned it to the foreman, the foreman had gotten bothered with him and told him to get back there and learn how to do it anyways, you're a machinist.
So Ed went to go operate the vertical boring lathe, it had no guards on it at that time. He reached in to brush some filings off and it caught his arm and pulled him into the machine.
It pulled him in, sheared him into pieces instantly and then spit him out on the wall behind. The company had the wall painted immediately after that.
The foreman who gave him shit and told him to do it anyway was immediately let go, however, the next week his car was at the shop across the road and he had no problem gaining employment.
Ed had a wife and child, he loved to play chess on break and will be remembered may you rest in peace brother
