Wrote up for missing work

Anybody have experience dealing with acquiring and using FMLA? My daughter has a seizure condition and I suffer from chronic pain. I was pulled in and wrote up for calling in sick or to care for my child (I’ve maybe missed four or five days this calendar year). I’ve contacted both my supervisor and manager, they will not return emails. Going through HR has been zero help either – I was essentially told that my condition and my daughter’s seizure disorder “did not qualify” for FMLA. When I brought up talking to attorney, I was referred to Absence One (apparently a third party that covers leave issues for some corporations) – I’ve attempted to deal with them via both phone and email – needless to say nothing has been resolved. Can I just get the FMLA forms from the DOL website and take them to my doc? Unfortunately I need…
