How to communicate to HR & manager about my CPTSD?

So I’ve had issues early on in my employment with my company from several call outs. All of them were due to my mental health, and my PTSD. I’ve called out 5 times in the nearly a year of working there. They don’t know I have PTSD because I feel like they wouldn’t believe me or think I am faking/exaggerating. Plus I have massive fears of them being mad at me for calling in. Or being perceived as lazy. I can only work part time due to said mental health complications. Discussing with my therapist , she said that I don’t have to tell them I have PTSD. But honestly I feel like it would be best to make it known. Our store is undergoing a massive renovation soon, which means I’ll be working while there’s jackhammers , drills, hammers, and other construction noises and often I get overstimulated/triggered in…

Our store is undergoing a massive renovation soon, which means I’ll be working while there’s jackhammers , drills, hammers, and other construction noises and often I get overstimulated/triggered in noisy loud environments. And it severely inhibits my ability to perform efficiently and normally

How do I handle this but also communicate with my employers in case of future issues with my mental health during this renovation?
