Misclassification of Independent Contractor.

So I worked for a technology repair company that repaired things for a large grocery chain. Half their workers were full-time and half “independent contractors” through a temp agency. We all had the same schedules, did all exactly the same work, they provided all the tools and equipment and we could not just come and go as we please otherwise we would be fired. I was going to file out form SS-8. But I wasn’t sure what company to put down. The parent company, the repair company, or the temp agency? Or is the answer whatever company I submitted my 1099 for. Also anymore advice? It also asked is the worker paid by another firm, would this be where I worked or the temp agency?
