I’ve been getting paid a dollar less than I negotiated for when I was hired mine months ago. What should I do about this?

To add some detail, I work in healthcare admin. In my interview I negotiated for a wage and was offered a probationary period at a dollar less, to be reviewed two months later. By the time two months had passed, the person who hired me was gone and their replacement declined to review me several times, saying that I was doing fine. At this point I frankly forgot that I had a dollar raise tied to this review and stopped persisting. Since then my department has shrunk from three people to just me and I am doing their work. I would not have taken this job at this offer. How do I approach this with my employer in a way that gets me closer to what I am worth?
