Do I need to put my corrupt supervisor on job applications?

Okay so, three months ago I got fired from my state job. I did not get any reason from my supervisor (she gave me none in verbal conversation or in my formal release letter) and HR refused to give me one. I've been desperately looking for work and they're all requiring that I put in my supervisor's name and phone number… I'm applying for food service/retail jobs. Let's just say this supervisor has lost wage theft lawsuits in the past and has a history of gross lying and misconduct. She also deadnamed/misgendered me my entire employment period. I had no other supervisor besides her. I did have another coworker that I would like to list as my “supervisor” but I'm worried about any background checks. How should I handle this….?
