Excuse for getting out of work a week before the scheduled shift?

One of my several jobs requires increasingly brutal 9-hour days, and it’s going to be v hard for me to make one of my shifts next week without huge amounts of stress (due to various responsibilities that day, as my family has a lot going on). Is it possible for me to make it to work that day? Sure. Is it worth it for the pay? Absolutely not. The day in question is 8-9 days away, so too late notice to randomly announce that I need the day off, as my boss is the only person who will be able to fill in for me. But I also really don’t want to fabricate an illness the night before/morning of, as it’ll cause undue stress for both me and my coworkers. What are the best excuses for needing a relatively-last-minute day off? The only thing I can think of is “I finally got in to see a doctor I’ve been on the waiting list for for months” though that in and of itself will inspire more questions than I’d care to answer.
