Has anyone ever seen a manager held accountable for their mistakes?

I have worked many jobs as a contractor and as a full-time employee. I have worked for some EPIC idiots. The best example I had was working for a corporate manager who barely had a high school education. His philosophy was that employees don't need training. His educated employees either figured it out or were fired. Needless to say, he was always complaining about his employees making mistakes. I had several conversations with him about his high turnover and his team's massive amount of errors. It was my job to help him get his shit together. My solution was to create a training program and allow his employees to ask questions. I didn't last long, but I always wondered how idiots get leadership jobs like this. Any thoughts?
