Help! In constant back pain but have no PTO to take time off.

I fucked up my back several months ago and have a physical job that involves a lot of lifting and carrying. The back pain keeps getting worse despite doing PT twice a week and taking it as easy as I can at work and at home. I started working here in May of last year but when the new year started I wasn't given my PTO because I hadn't been here a full year so I have to earn it at a snails pace. I've already used up all the PTO I've earned this year and taken a few unpaid days for Dr appointments and days where I couldn't handle the pain, and my boss is getting on me about it and saying I could lose my full time status if I don't work enough hours for the year, and my insurance is provided by my job so I need…

I want to ask for just the rest of the week off to give my back a little time to heal because I haven't had more than two consecutive days of rest since I hurt it. I'm worried my boss is going to react very negatively to my request and I really just fucking hate everything right now. If anyone could offer some advice I would greatly appreciate it.
