High level job and burnt out from chronic illness

I am in a decently high level job for a US company with about 30,000 people. Even though we fully teleworked for the last 2.5 years they want people to come back into the office on a “hybrid” schedule. I developed some unexpected chronic illness last Winter and have felt unwell every day for 8 months. I had some time off of work for medical leave but my claim was denied so I burned through sick time and paid time off. They also burned my FMLA despite my using my own time. I am going to appeal it but I don’t understand. I’m basically burned out and sick. Unfortunately, I live in the US and I don’t have a clear diagnosis of what is going on and don’t qualify for any ADA protection. I am sick and tired and can’t keep working a job that requires a team of people…

I am sick and tired and can’t keep working a job that requires a team of people and it’s just me. I keep saying I need some support.

I’m stupid and bought a house last year. Then I got sick. So now I just do what I can each day so I can pay my mortgage and maintain my healthcare.

I have not had a vacation in years.

They said they would let me telework for 6 more months because I have a detailed account of my illness to my boss and then had an attorney help me fill out the form to request an accommodation.

If I can’t telework I can’t work. I’m trying to find energy to pack up and move to get out of this mortgage but rents are so high it would be equivalent to my mortgage payments.

I’m in my forties and have no idea what I am going to do. I only know I wake up with dread every day about my job and can’t take it anymore.

They expect us to work from 8am to about 6pm each day and crank work out like crazy. I just can’t anymore.

I need a break but I’m concerned about using time off because I need it for doctor’s appointments. Everyone has been on vacation. I can’t keep doing this.

Any ideas?
