How do other people with homebound parents take care of them.(former landscaper,cook)

I woked hard in fast food in New England after leaving NC when I was young. Worked farming jobs,cooking cleaning, and landscaping. There are ZERO programs here in NC. I had to move back after my dad got too sick to care for himself. We're on the coast so our house is on pylons. It's impossible for him to get down there stairs. My dad has copd,diabetes, wheelchair bound and has weight issues. He also has early onset dementia. Hes only 70. There is nothing in place to help us. I can't work because I have to help him into the chair when he falls. Which happens a lot. I have to Cook,clean and make sure he doesn't fall down again. The local emt has to take him down the stairs for the few appointments he has. I can't work because I'm here 24/7. Is there a program I'm missing…

I can't live like this. I cant leave him also . The nursing home here costs almost $3000 a month. I'm desperate. What can I do. I'm drowning here.
