how to avoid answering salary history request?

I'm about to interview for a state/city job in Georgia, US (a state that gives 0 fucks about employee rights, think at will state), and the interviewer asks that I bring in forms to my interview. One of these forms is a salary history, I'm asked to fill out my job title, my time there, and my pay. It's not illegal in my state to ask this but I'm not obligated to give it to them. I have been seriously underpaid before as a woman and a freshly graduated student and do not wish that to reflect my potential salary. Is there a cordial way to go about denying them that information without risking the job opportunity? Thanks!
