how to talk to someone who is working themselves to death?

My mother in law works at a Cafe in the city where she is underpaid and under appreciated. She has multiple health problems and her boss is a dick, forcing her to miss specialist appointments to work, even though she is casual. She has been there for 8? years and should have been offered full time and hasn't even been offered a raise. She is the only staff member and she likes her clients, and has a misplaced sense of obligation to run the cafe for them, even though the city is full of cafes they could go to instead. How do I convince her to stop working until her health problems are fixed and to look for another job?
