I asked for disability accommodation to wfh, they sent me FMLA forms to go on leave? Anyone know what bs this is?

Per my last post here, my employer is trying to make me go into the office twice a week starting next week. They pay me so little, the commute would genuinely fuck me, but that’s also not the point. I’ve been wfh since I started with them and have been a top performer in the company. Even our research & development department uses my reports as examples for nationwide training. I’ve only been here a year and some months, HR calls that “a long time”. Anyway it kinda sucks and I’m too good for this. I’m interviewing somewhere else and hope to leave this place in the dust, but for the time being, I need to act like I’m stuck here. I asked to initiate the interactive process for wfh accommodation for my ADA-recognized disabilities. I have a hard time believing anyone will be able to say that it’s “undue hardship” not to accommodate, given that I am only asking for nothing to change from my current work environment. They sent me the forms for my doctor to fill out, and one of them says I am intending to go on FMLA leave for a medical condition starting 10/1. I am ???? because I said nothing of that. I told the HR rep this, and she insisted they were the right forms. I have asked for accommodation with an ADA form, but haven’t worked a job that’s eligible for FMLA before, so maybe I’m missing something. My doctor’s appt to get this process started is this week. I’m hoping she will know what this means. But in the interim, I wanted to ask this sub whether they’d seen this before. It seems like they might be trying to underhandedly fire me for refusing to come back into the office, and possibly trying to confuse me on purpose bc I am saying I’m disabled.
