I believe my boss is committing insurance fraud and I need help defining it

Hello, so my boss is a doctor who owns his own medical practice, and something I've seen him do is schedule a patient on a day they did not come in after a day they actually did come in. This is done to ensure he gets paid for the services he provided and the ones that another physician provided. It was explained to me that he won't get paid for both services provided by him and the other physician because you can't bill two providers at once. Is this insurance fraud? He also has our biller dictate his doctor's note; is that illegal too? Im considering reporting him, and I need to know what type of fraud he is committing. He is a shady man and a pathological liar, and I don't want him getting away with the shady shit he does anymore. Sorry if this is not the right…
