I need help – My coworkers and I are trying to get the attention of our higher managers with a survey from our customers.

Hello! Long time lurker, first time posting. The company that I work for has been awful to us in various ways. I'm trying to set something up where we can give a customer a link to send feedback to our higher mangers to their work email. I've been looking around for a few hours trying to find the best way to do this but have come up with nothing. Ideally, it would just be a single page with a text box where they can send feedback to multiple email addresses (both of the managers). Has anyone done anything like this before? ​ Sorry for the formatting or grammar, I'm writing this quickly. Thank you!
