Medical accomodations for job where job duties changed

Posting for my mom. She's been working at the same place for 10 years, and part of the job description is “lifting up to 20 lbs”. Recently tho, she's been expected to regularly lift 40+ lbs. She's 64 and has trouble lifting that. None of her coworkers are expected to lift the 40+ lbs. Her employer hasn't asked her to agree to a change of job description, but is now asking her to get medical accomodations to be exempt from the new tasks. She can't afford the medical bills to go thru the process to get a drs note. What next?
