Medical leave is unscheduled unpaid leave?

My wife works almost full time and I'm a stay at home dad. I have a 2 year old daughter and my wife is 7 months pregnant however our unborn son is having medical issues and in order to have a safe delivery we have to travel 4 hours to a very good hospital so that they can do the surgery. All in all it's a little stressful in terms of travel times but it sounds like this case is incredibly mild and shouldn't be a whole lot of trouble in terms of survivability for our son. All in all it's a little stressful in terms of travel times but it sounds like this case is incredibly mild and shouldn't be a whole lot of trouble in terms of survivability for our son.

Unfortunately, because of the long distance to the hospital we need to go to, and because of the volatility of the way they work, they keep giving us a week or two notice for our next appointment, and my wife is a teller at a federal bank. My wife has already missed a lot of work so far due to the extremes of her pregnancy and also because of the sudden and frequent doctors appointments and because of that she is out of PTO.

So this next appointment which we heard about yesterday and is happening 7 days from now and is causing her to miss two days of work is causing my wife's boss to stress about being understaffed and overworked during the times that my wife is gone. Don't get me wrong, my wife's boss is a fairly nice lady, and she's my aunt (related by ring). But she said that “5 unscheduled and unpaid absences effects [my wife] negative.” She didn't explicitly say that she'd be terminated or anything like that. But I thought if you gave a doctor's note it became an excused medical absence or something. I just find it strange she's getting reprimanded for a situation that's mostly out of our control.

Also to clarify, we have talked to the hospital about giving us more notice and they seemed to mostly shrug it off.
