Old man with questions about PTO and Paternity

Wife is planning a C-section sooon. I planned to take time off for paternity. But as I'm doing this, I realize I'll have to pay benefits (medical 401K, etc) while I'm out. It's also a crazy process involving the state, an insurance company, etc. So I look into my PTO balance and learn that we're one of those “unlimited” PTO gigs. I request the three weeks off. I'm told PTO is different from leave of absence. So my question is – is it really? One costs me more, and takes a much longer process to go through; the other is completely discretionary. Am I wrong to just ask for the time as PTO?
