Potential employer asking about birth control

Around a year ago, I applied to work at a major NYC hospital as a research assistant. There were a few red flags throughout the interview process, but the nail in the coffin was when they asked me to come in for a medical screening. These are pretty standard in hospital/healthcare jobs, but I was extremely put off when they sent me a form to fill out that wanted me to list previous medical history and all of my medications, including type of birth control! I’m happy to take a TB or drug test, but I refuse to disclose to my employer what type of contraceptive I take, or even if I take one at all. I turned down the job offer, but I’m wondering if this has happened to anyone else? Never heard of employers asking about birth control status before.
