“Sign-on” bonuses: just a big trap?

I'm seeing a lot of job postings offering sign-on bonuses (up to $20,000 for some like nurses or pharmacists) and am curious what the rub is here: are they just a way to lure good workers into a crappy work environment, then find a way to force them out before the contract ends & require repayment of the bonus money? It seems like a red flag, but I've only heard bad news. Anyone have positive experiences being offered & given your entire sign-on bonus?
