Took unpaid leave for four business days from my call center job due to what I know is burnout now( more explained in post), what should I do next?

A few weeks ago my workload got bigger when we got added to another system. When this new system came out, my QA was fairly still good. I got two kudos within a week then on Wednesday of this week, I got a 90 on a call because I didn’t “show empathy” or the call was transactional. Get this, I never have had an issue with QA taking the points off a call because of those things before. We have to try and maintain 97% adherence as well as 97% on QA. I sent an email to my supervisors Thursday morning about the issues about my productivity and I have not been on my work computer since. I have a resume coach revamping my resume and she is helping me get higher paying jobs that will get me off the phones. I have been struggling in my personal life as well such as moving to a new city not too long ago, trying to keep a social life, real estate school, etc! This week I literally had to push myself through Monday-Wednesday. Thursday I logged in and told myself “I can’t work today”. I called my work yesterday and called HR to use short term disability and took off until the day after Thanksgiving. The thing is I like this job. I just have 5+ customer experience and a communications degree and I’m trying to get into real estate. I am trying to get a data entry or chat and email support job next and that is what my resume coach is helping me with! I also have a phone call with my burnout coach this Monday night me this is motivating me a bunch! What should I do about my current job for the time being? Start looking elsewhere or stay and try to see if my supervisors will accommodate me? My burnout coach said I’m making notice of my burnout and taking action most people don’t admit that it is happening to them. I am trying to be proactive as possible.
