What’s the best way to tell my boss I’m leaving so that they won’t keep me any longer?

I know it’s partially my fault because I keep asking them if I’m “good to go” and they usually give me another task to do before I leave. But I actually have places to be in less than an hour after work and I don’t want to stay after my scheduled hours. A lot of my coworkers are very young teenagers who don’t realize that pizza parties are not adequate incentives for being “team players”, so management expects the same from me. What’s the best way to get across that I’m leaving without potentially getting disciplined?
