Which CEOs directly cost their company more money than they were worth?

Title really says it all, I am curious to see which CEOs cost their company more money than their salary was worth based on company performance via their direct decisions. Could be just over time or as a result of a specific decision, like switching a product or failing to upgrade with the times, am curious as I suspect CEO performance is as random as a monkey throwing darts to pick winning stocks
