why does in home care cost so much but pay employees so little ?

While I understand that companies have to have a positive revenue. My job involves in home care for seniors the monthly cost for 1 senior varies from 2300-4500 a month but I get paid 12.50 an hour to go in and literally take care of them (wash them, feed them etc) on top of you don't get benefits until after a year which it only covers preventive procedures. I don't mind the job and I enjoy helping seniors but I see why people don't want to work for these companies. I would get the pay if this was government funded or for low income families but this company only aids those who are rich. I guess it is just mind blowing to me that they wouldn't pay people more but wonder why they struggle to keep people on board.
