Can you fine folks help me word a certified letter to a former employer? (Context inside)

I worked at a local doctors office for 2 weeks in total. The place gave me about a million red flags so I cashed my first check, wrote my resignation effective at the end of that week, and then waited patiently for 2 weeks after leaving to receive my second check, since I entered halfway through their billing cycle. It’s been months with no check, and I just received my W2 from them confirming they still owe me a couple hundred bucks from that last week. I’m actually terrified to return to the place to demand the check in person, so my grandfather suggested a certified letter requesting the paper check (because this office refused direct deposit right out the gate) Can you help me word a letter so that I don’t end up sending a certified letter that just says “pay up bitches.” I’ve never had to do something…

Can you help me word a letter so that I don’t end up sending a certified letter that just says “pay up bitches.” I’ve never had to do something like this before, so thanks in advance, I appreciate it a lot!
