This post is a call for news stories in which corporations have been documented to ignore ethics/safety advice and warnings. The more modern the cases, the better.
I hope that this is an appropriate forum to post this in. The reason I am asking for this list is that I am creating a course, and would like to have some topical cases in which corporations simply ignored safety advice in the pursuit of profit. E.g. Examples of how companies do not self-regulate, or at least often do not, and even ignore the people they supposedly hire for that purpose.
Of special interest is if ethics boards or consultants are disbanded after they raise an alarm. Also of special interest are cases where workers or consumers end up harmed as a result.
To give an example of what I mean, this recent article (https://www.nytimes.com/2023/04/07/technology/ai-chatbots-google-microsoft.html) is about Microsoft and Google ignoring their own safety advice. Though this particular case hasn't had any massive consequences yet it shows them ignoring the ethicists that they themselves hired.

I do ask that any example come with a link to a legitimate news source that documents the case.
