Coworker keeps asking me to cover shifts for him.

We're both bartenders and I've recently relapsed with my alcoholism. Until I get my 2 week LOA for therapy I'm already struggling to be at work at all. He keeps asking me to cover his shifts and at this point I just can't. I asked him to cover a shift or two of mine back when I was sick, and he just ghosted me. Now he's asked me 3 separate times to cover for him, and on top of my relapse issues, I just can't bring myself to help out. After the first couple times when he asked me for cover, I had the decency to at least respond… now I'm just ghosting him back because I feel awkward saying no every time. I don't want our managers coming at me for never covering for him, but they know the gist of why I'm taking an LOA in the first…

I don't want our managers coming at me for never covering for him, but they know the gist of why I'm taking an LOA in the first place so I don't feel it would be fair for me to force me into something I'm struggling with. Especially after I've asked to be moved back to the dining room many times so I'd only have to serve wine. I also don't feel comfortable disclosing that I'm an alcoholic to this coworer.

Is this shitty of me? What can I do?
