Having a bad case of insomnia, no sleep in 40 hours. Thinking about calling out but feeling nervous

I started on 3/17 and have taken PTO on 4/1, 5/17 & 5/18, and 6/14…. That's 4 days already, will using a PTO day tomorrow be too much or do you think i'm on the radar? We get 15 days throughout the year but just not sure cause still new….
