help writing accommodation request

I need a raise & boss will give me one but I cannot meet the reduced time requirement(1.40) with the same high quality because of my nerve damaged hand. I need 2 hours, which I currently get as a junior. I have asked for a reasonable accommodation due to a severed ulnar nerve. Basically my left hand has about 50% function from a car accident in '03 (it's why I had to stop cutting hair) The Accommodation would be for me to keep the same service time of 2 hours but get the raise to senior. 10$ hourly + better commission They want me to have a doctor Note explaining why I should get the accommodation (but have given me no guidance) so I'm using some reasonable accommodation forms online. I'm filling most of the parts so my doctor can just read and sign. I'm really the one who knows…

So what I am looking for are some professional ways to say I am capable of doing this work despite this medical disadvantage as I don't want them using this as an excuse to fire me.
I like my job, I'm #1 in sales and always have a full book.
