How do I communicate with my boss wothout appearing to quit?

I hope this is the right place to ask this, but I need some help. Hopefully it doesnt turn to ranty and people can understand my situation. I'm in the US and I've been out of work on FMLA since early November. My time has run out, and I finally have a diagnosis, fibromialgia (amoung some other things, but thats the main one). Even though I finally got a diagnosis, I'm definetly not well enough to return to my very physical maintenance job now if ever. My doctor says it's most likely out of the question to do anything physically intensive for work even if I was recovered from the last few months. The problem is, my boss loves to screw people over. The problem is, my boss loves to screw people over. I work in a school district and they already had planned on bringing me back to very different hours, at a different location completely against my will, which feels like they want me to quit. For example, i used to work 6am to 2pm, they want me to work from 12 to 8pm. They decided this the day i announced i was going on leave, and have continued to make it unattractive to come back. I'm trying to write and email telling them that I can't return, but even I can see in the way it's worded how they could twist it to say I quit instead of being terminated. I really want to get fired in case there's an issue with getting disability, I'd like to be able to temporarily fall back on unemployment if I need to until I'm well enough to at least do an office job. Any advice would be hugely appreciated.
