Is my PTO policy as bad as I think it is?

I thought I got 80 hours (10 days) of PTO, but my boss just told me that I only get 40 hours (5 days) because I “only” have been at my job for 1 year. This includes sick days, so if I take 3 sick days, it counts as 24/40 hours. On my 2-year anniversary, it goes up to 80 hours, and after 7 YEARS, it goes up to 120 hours (sick days are also included in these). If we don’t use our PTO, it’s forfeited. It doesn’t roll over. Paid maternity/extended leave isn't offered. “Under certain circumstances” we can take 6 weeks of unpaid leave if it’s approved by management. And we get 5 paid holidays – New Years Day, 4th of July, Labor Day, Thanksgiving Day, and Christmas Day. I’m getting surgery in early 2023 and I'm stressed. I’m getting surgery in early 2023 and I'm stressed. My doctor recommends taking 10-14 days off work. But I'll only have 5 PTO days. So I either have to return to work too soon, or not get paid and not take any other vacation/sick days for the entire year because I won't have any left.
