Leaving gracefully after repetitive motion injury

I started a new laboratory job that requires repetitive motions. There is a 3 month probationary period before benefits kick in. I have been here a month. I have a tendinitis in my elbow and wrist. Since working this job it has spread to my thumb and finger from pipetting. This is not a job I want to continue. I have an annual doctors appointment with my pcp in two weeks but the pain is such where I am thinking I will need to go to urgent care today. I know they will recommend at the very least rest. My question is how do I gracefully say that given my injury, I cannot continue working. Even data entry is painful. I’d like to give two weeks notice but the pain I am experiencing is triggered even picking up a mug of coffee and severely limits what I can do in…
